  1. LI observed Resident 4 in bed with bedrails in the up position. Resident 4’s Individualized Service Plan, developed 7/18/2024 and reviewed on 8/18/2024, did not identify the resident’s need for bedrails.
Plan of correction
Resident 4 Individualized Service Plan with ALD and POA. ISP updated to include side rails as an assistive device for turning and repositioning while in bed when staff is in the room and care is being provided. Side rails are to be lowered when not in use when resident is in bed. Additional review of resident files completed to ensure assistive devices are appropriate noted in the care plan. Education provided to direct care staff on the appropriate use of assistive devices.

Date of Inspection: November 9, 2023 Type of Inspection: Renewal inspection Census 39 Number of records reviewed and interviews conducted- 6 records, 4 interviews. All facility self-reported incidents since the last inspection were reviewed on this date. The LI observed residents participating in independent activity pursuits, going on a community sponsored outing and eating lunch. This Licensing Inspector also observed medication administration and compared the physician order sheets to medications available on the medication cart. Staff B and C were corrected the same day of inspection. Additional inspection of facility staff of other staff files was found and corrected immediately. Future plan is to complete risk assessments every January of all staff members regardless of the start date to ensure that the risk assessments are complete.
