- On 8/16/25 during an altercation between resident #1 and staff #3, staff #2 and staff #3 began to record the incident with their cell phones.
- Staff #1 confirmed that both staff #2 and staff #3 recorded the resident-to-staff staff altercation.
- Staff #1 also confirmed that staff #2 and staff #3 did not follow facility policy in reference to prohibiting the use of photographic and audio devices.
- It was confirmed by Staff #1 that staff #2 and staff #3 both received a written disciplinary action and were both taken off of the Memory Care Unit.
- Staff #2 and staff #3 were also required to retake training on Resident Rights and Mandated Reporter training. ;
