Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of these inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. Based on record review and staff interview, the facility failed to implement a portion of its medication management plan, specifically, regarding its methods to prevent the use of outdated medications.
Evidence
- During an audit of the facility’s medication carts on the date of inspection, LI observed a box which contained a bottle of LATANOPROST 0.005% eye drops belonging to resident 10. The box contained an open date of 02/28/2026 and an expiration date of 04/11/2026. Instructions for the use of LATANOPROST 0.005% indicate that the eye drops can be used for up to 6 weeks after the date that the bottle is opened.
- A review of the facility’s most current medication management plan, dated 02/20/2025, indicated on page 2, section 3, point 2, that “Any expired medications must be removed from the medication cart and disposed of immediately by the med person”.
- An interview with staff 5 on the date of inspection confirmed that the LATANOPROST 0.005% eye drops that were in the cart were expired based on the opening date that was written on the box. Staff 5 visually confirmed with LI that an unopened box of the LATANOPROST 0.005% eye drops for resident 10 was available in the refrigerator in the medication storage room.
Plan of correction
On the date of the inspection, the out of date bottle of Latanoprost eye drops was
removed immediately from the cart and properly disposed of and a new
unopened bottle was obtained for administration.

The Director of Nursing will reinforce the medication management procedures in
accordance with regulation 640-A. All registered medication aides and Licensed
nursing will be re-educated on the methods to prevent the use of outdated
medications and complete weekly audits of all nursing carts. DON will review
audit sheets for trending and ensure compliance with medication management
requirements.
