The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility.

Observations by licensing inspector: The Licensing Inspector observed the residents during activities and meals. The Licensing Inspector reviewed the following at the time of inspection: fire drills, emergency drills, resident council reports, pharmacy review, healthcare oversight, menus, activity calendars and dietician report.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Based on record review and staff interview, the facility failed to ensure that fire and emergency
evacuation drill frequency and participation were in accordance with the current edition of the
Virginia Statewide Fire Prevention Code (13VAC5-51).
Evidence
- Record review of fire drills completed since July of 2024 showed the following drills completed; 7/13/2024 first shift, 8/14/2024, 9/31/2024 and 10/30/2024 second shift, 11/27/2024, 12/18/2024, 1/31/2025, 2/28/2025,3/31/2025 all first shift, 4/30/2025 second shift, and 5/30/2025 first shift.
- Upon request the facility did not provide any third shift fire drills.
- During an interview with staff 1 and staff 8, when asked if the fire drills were completed as required staff 8 stated “no, they were not”.
Plan of correction
On 6/9/25, ED (Executive Director) audited the last 12 months of fire drills with no additional findings noted. On 6/17/25, the Maintenance Director conducted a third shift fire drill. On 6/9/25, ED educated Maintenance Director on requirement for conducting fire drills monthly on a rotating basis to include first, second, and third shift. Staring 6/9/25, ED will audit fire drill documentation monthly x12 months to ensure ongoing compliance. Results of the audit will be discussed at monthly QA meetings and will cease after three months consecutive compliance.

Based on record review and staff interview, the facility failed to ensure that the criminal history record reports were obtained on or prior to the 30th day of employment for each employee.
Evidence
- Record review of staff 9 (hired 2/17/2025) had a criminal history record report dated 3/26/2025 and staff 10 (hired 3/25/2025) had a criminal history record report completed 5/13/2025.
- During an interview with staff 1 and staff 11 when asked if the criminal history record reports were completed on or prior to the 30th day of employment staff 11 stated, “no, I have no idea what happened with those.”
Plan of correction
On 6/9/25, the ED (Executive Director) and BOD (Business Office Director) audited current employee files with no additional findings noted. On 6/9/25, ED educated BOD on requirement for obtaining criminal history record reports on or prior to the 30th day of employment for each new employee. Staring 6/9/25, BOD will audit new hire documentation monthly x12 months to ensure ongoing compliance. Results of the audit will be discussed at monthly QA meetings and will cease after three months consecutive compliance.

Based on record review and staff interview, the facility failed to ensure the written Do Not Resuscitate (DNR) order for withholding cardiopulmonary resuscitation from a resident in the event of cardiac or respiratory arrest is included in the individualized service plan (ISP)

- Resident 1 admitted 6/9/2024, had a DNR order dated 6/22/2024, the ISP for resident 1 dated 6/8/2024 did not include the DNR order.
- Resident 5 admitted 4/17/2024, had a DNR order dated 4/11/2024, the ISP for resident 5 dated 4/17/2024 did not include the DNR order.
- Resident 6 admitted 6/1/2023, had a DNR order dated 2/14/2023, the ISP for resident 6 dated 3/22/2024 did not include the DNR order.
- Staff 2 stated during an interview, “I don’t see it” when asked if they could locate the DNR order on the ISP.
