August 7, 2025Inspection
- During a tour of the facility on 08/07/2025, at 1:10 p.m., the licensing inspector (LI) observed the kitchenette, located in the Bridges neighborhood, had the following items unsecured: a) The cabinet under the sink contained 1 can of Raid max ant and roach spray. b) The second cabinet contained 1 quart spray bottle of liquid comet cleaner, 1 spray can of OdoBan disinfectant air freshener, and 1 clear spray bottle with an unlabeled liquid.
- During a tour of the facility on 08/07/2025, at 1:20 p.m., the LI observed the Physical Therapy room was not locked, and the following items were found unsecured: a.The wall cabinet contained 4 cans of biofreeze medical spray. b. The bottom cabinet contains a I-quart bottle of Clorox bleach germicidal cleaner, 1 Lysol air freshener, and 3 clear plastic spray bottles with an unlabeled liquid.

- On 10/16/2024, at approximately 3:50 pm, the licensing inspector (LI) observed the medication cart, with a computer on top of the cart, in the facility’s kitchen and dining room area. The LI observed the computer screen open with resident information displayed.
- The computer screen displayed 16 residents’ photos and room numbers.
- Staff 1 confirmed the medication cart computer screen was open to public view.

July 25, 2024Inspection
- In the kitchen walkway area, there were floor tiles that were lifted up and damaged. 2.The floor tiles were lifted up in a manner to be a potential tripping hazard. 3.Photos taken as evidence.
- The facility did not have a drug reference book on the medication carts or in the medication/Nurse’s station.
- Staff 2 was asked to locate the drug reference book but was not able to do so.
- Resident 1’s ISP does not indicate the need for a “STOP SIGN” velcro banner across their door.
- Resident 2’s ISP does not indicate the need for a “STOP SIGN” velcro banner across their door. 3.Staff #1 stated that the “STOP SIGN” usage is to keep wandering resident(s) from entering the rooms of Resident #1and #2.

July 27, 2023Inspection
- LI conducted a renewal inspection on 7/27/2023 and observed the following:
- For Resident #5, a physician’s order written on 5/26/2023 for Ibandronate Sodium 150mg evidenced the medication is to be administered “once a month on the 14th.”
- The July Medication Administration Record (MAR) for Resident #5 documented that Ibandronate Sodium 150mg was administered on 7/13/2023 at approximately 8:00 am by Staff #1.
- LI conducted a renewal inspection on 7/27/2023 and observed the following:
- For Resident #3, a progress note dated 3/27/2023 documenting that a new physician’s order had been received by the facility ordering that “HCTZ (hydrochlorothiazide)12.5 po daily-hold for SBP (systolic blood pressure) below 110.”
- The July 2023 MAR for Resident #3 evidenced the following: “Hydrochlorothiazide 12.5mg cp (capsule) Take one capsule by mouth every day for hypertension.”
