A non-mandated self-report monitoring inspection was initiated on 12/17/2021 and concluded on 12/20/2021 by Licensing staff. Licensing Inspector conducted an onsite inspection of 12/20/2021 from 11:20 a.m.-12:30 p.m. COVID screening questionnaire completed. The Director of Wellness and the Assisted living Coordinator were interviewed. The resident was observed and interviewed. Resident's room, and care notes were reviewed along with individualized service plans and uniform assessment instrument. The evidence gathered during the investigation did not support the allegations self-report/complaint of non- compliance with the standards or law.

A renewal inspection was initiated on 8/12/2021 and concluded on 8/18/2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census is 142. The Inspector emailed the Administrator a list of items required to complete the remote documentation review portion of the inspection. The Inspector reviewed 5 resident records, 5 staff records, staff schedules, sworn affidavits and criminal back ground checks on new hires, physician's orders, medication administration records, fire and health inspections, health care and special diet oversights, fire drills, activity calendar and menus submitted by the facility to ensure documentation was complete. The Inspector conducted the on site portion of the inspection on 8/18/2021. An exit interview was conducted with the Administrator and Wellness Director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
