- On 05/12/2026, Resident 2’s (admitted 01/11/2023) ISP dated 03/16/2026, was not signed by the resident or their legal representative.
- Staff 1 and staff 2 acknowledged the LI’s findings.
- On 05/12/2026, the Licensing Inspector (LI) observed that floor tiles in Resident 4’s bathroom had shifted away from adjacent tiles, leaving visible gaps in the flooring surface. The LI also observed gaps between floor tiles in Resident 2’s bedroom area.
- The LI observed that the white veneer on Resident 4’s bathroom cabinet was peeling around the top edges near the toilet paper holder.
- In the kitchen, the LI observed that the bottom white cabinet drawer closest to the stove was hanging from one hinge and was not maintained in good repair.
