Based on record review and interview, the facility failed to ensure the Individualized Service Plan (ISP) was signed and dated by the licensee, administrator, or their designee, and the resident or their legal representative.
Evidence
  1. On 05/12/2026, Resident 2’s (admitted 01/11/2023) ISP dated 03/16/2026, was not signed by the resident or their legal representative.
  2. Staff 1 and staff 2 acknowledged the LI’s findings.
Plan of correction
All resident Individual service plans were audited on 5/12/2026 for responsible party signatures. Administrator & Manager will audit every three months and or as needed moving forward

Based on observation and staff interview, the facility failed to ensure that the interior and exterior of all buildings were maintained in good repair.
Evidence
  1. On 05/12/2026, the Licensing Inspector (LI) observed that floor tiles in Resident 4’s bathroom had shifted away from adjacent tiles, leaving visible gaps in the flooring surface. The LI also observed gaps between floor tiles in Resident 2’s bedroom area.
  2. The LI observed that the white veneer on Resident 4’s bathroom cabinet was peeling around the top edges near the toilet paper holder.
  3. In the kitchen, the LI observed that the bottom white cabinet drawer closest to the stove was hanging from one hinge and was not maintained in good repair.
  4. Staff 1 confirmed the LI’s findings.
  5. Photos were taken as evidence.
Plan of correction
On 5.12.26 company maintenance provider was contacted regarding interior building findings. Provider came out on 5.13.26 to correct & fix. Moving forward administrator, Manager, and Maintenance Provider will audit building’s interior/exterior to maintain in good repair.

An unannounced mandated monitoring inspection was conducted on 5/24/2023. At the time of entrance eight residents were in care with two staff providing care and one staff conducting activities. The sample size consisted of three resident records, three staff records and one individual interview. Resident and staff records and other documentation were reviewed. Virginia State Police background checks reviewed for all new staff hired since the previous inspection conducted on 5/5/2022. Residents were observed eating breakfast and engaging in activities including current events and exercise. Medication administration was reviewed. The Building, Fire and Health Inspections have been submitted and reviewed. The facilities Policies and Procedures were submitted electronically and have been reviewed and approved. It should be noted that the new owner maintained all of the previous owner’s policies and procedures. The Fire Evacuation and Emergency plan has been approved by the local Fire Marshall. The Licensing Inspector was last in this facility on 8/10/2020 and no physical plant changes have occurred since that time. The resident room and window dimensions have been verified. Please note that per the Building Official no more than 5 non- ambulatory residents may be present at any one time. No violations cited today.
