Plan of correction
Administrator will ensure that designated staff member and residents and or their responsible party will review, sign and date all service plans as they are completed. Administrator will have all current documents reviewed with residents and responsible parties,signed, dated and placed in each resident's chart.

Based on a review of resident’s records, one record out of three did not have the required tuberculosis test form completed.
Evidence
  1. Resident #2 was admitted to the facility on 3/19/2022 and there was no documentation of a completed TB test. The form was not completed.
Plan of correction
Administrator will ensure the TB test is completed and the results will be placed in the resident’s file. Designated staff will ensure that all admissions documentation are received and completed upon admission and annually thereafter.

A monitoring inspection was initiated on 9/14/2021 and concluded on 9/17/2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 5. The Inspector emailed the Administrator a list of items required to complete the remote documentation review portion of the inspection. The Inspector reviewed 2 resident records, 2 staff records, staff schedules, menus and activity calendars, fire and health inspections, fire drills, physician's orders, medication administration records, health care oversights, submitted by the facility to ensure documentation was complete. The Inspector conducted the onsite portion of the inspection on 9/17/2021. An exit interview was conducted with the Administrator and Manager on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
