The evidence gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation will be addressed in order to return the facility to compliance and maintain future compliance with applicable standards or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. Activities -suggestion offered on activities for individuals in care. Licensing staff will offer resources to assist in enhancing activities offered and provided.
Comments
A on-site complaint inspection was conducted at the facility on 3/11/2011. A complaint was received by the department regarding allegations in the area of Resident Care and Related Services. Licensing staff reviewed the resident file (medication records, physician orders, hospice notes) and interviewed staff and inspected the activity schedule. The evidence gathered during the investigation did not support non-compliance with standards or law.

A monitoring inspection was initiated on 8/28/2021 and concluded on 8/31/2021. The (person in charge) was contacted by telephone to initiate the inspection. The administrator reported that the current census is 8 residents. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two staff resident records, two staff records, required postings, medication administration and inspected the building to determine compliance. The inspector conducted the on-site portion of the inspection on 8/31/2021. An exit interview was conducted with the Administrator by telephone and email on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the facility.

Based on an on-site inspection of the building on 8/31/21, the building is not maintained in good repair and kept clean. The outside of the building was observed to have cobwebs around windows and doors. Paint was observed to be chipping around windows.

Cobwebs to be removed. Maintenance has been previously scheduled, contractor delayed work due to COVID.
