- During the interview with staff #1, they informed the licensing inspector that between 4:00 PM on October 16th, 2025, and 6:00 AM on October 17th, 2025, resident #1 was not transferred via 2 staff as specified in the service plan for resident #1.

- The facility submitted a self-report on August 11, 2025, stating resident #1 was at an area hospital. The facility stated that prior to the hospital calling the facility, employees were unaware that resident #1 had eloped.
- Employee #1 confirmed during their interview that resident #1 had eloped and that prior to the area hospital contacting the facility, the employees were unaware that resident #1 had eloped.
