- Resident #1's record contains an order for Novolog, that calls for the medication to be administered dependent upon the resident's blood sugar on the following sliding scale: If 150 - 199= 1 unit call if less than 60; 200 - 249= 3 units; 250 - 299= 5 units; 300 - 349= 7 units; 350 - 399= 9 units call if greater than 401 Resident #1's Medication Administration Record (MAR) indicated that his Novolog was not administered on 5/5/26 (noon administration) when his blood sugar was measured as being 155. The MAR documentation for the administration was coded as "no insulin required." No additional notes were provided, during the inspection, to clarify why Resident #1's Novolog was not administered on the above date. Facility staff confirmed that no additional notes were present about the Novolog administration on 5/5/26 (noon administration).
- Resident #1's Venelex was observed during the inspection. The tube indicated that the Venelex expires on 06/26. The tube also states "see package insert for full prescribing information." No package insert or pharmacy container (with the prescription label) were observed for Resident #1's Venelex, to look for additional details about the exact expiration date. Staff #1 confirmed that Resident #1's Venelex package insert and pharmacy container were not present, at the time of the cart inspection.
