Based on review of personnel files and staff interview, the facility failed to ensure unlicensed Resident Assistant's (RA's) were trained and successfully signed off by a Registered Nurse (RN) or Licensed Practical Nurse (LPN) as required. This had the potential to affect all 83 residents residing in the facility. The facility census was 83.
Findings include:
Review of RA #158's personnel file revealed the staff was hired on 03/19/25. Further review of RA #158's personnel file revealed document for technique training for resident personal care dated 07/09/25 which was signed by RA #144.
Interview with Executive Director (ED) 09/11/25 at 11:05 A.M. revealed the initials training and signing off RA #158 on resident personal care technique was RA #144. ED confirmed RA #144 was not a licensed RN or LPN.

Based on staff interview and review of fire drill reports, the facility failed to ensure residents were evacuated in at least two fire drills a year on each shift. This had the potential to affect all 41 residents residing in the facility. The census was 41.

Review of the fire drill reports from November 2023 to October 2024 revealed resident evacuations only occurred on one second shift fire drill in June 2024.
Interview on 11/14/24 at 2:06 P.M. with Maintenance Director (MD) #104 verified evacuations had not occurred in at least two fire drills a year on each shift. MD #104 reported he had been doing evacuations on two disaster or fire drills in a year.

Based on observation, staff interview, and policy review, the facility staff failed to store foods in a safe and sanitary manner. This had the potential to affected all 28 residents residing in the facility. The census was 28. Findings include: Observations on 07/15/24 at 10:17 A.M. of the foods stored in the walk-in kitchen refrigerator revealed a pan of chicken salad dated 07/04/24, chocolate cheesecake filling dated 06/30/24, and coleslaw dated 07/07/24. Interview at that time of discovery with Dining Services Supervisor #30 verified the foods were prepared from scratch and stored since the respective dates documented on each food item. Interview on 07/25/24 at 10:20 A.M. with Food Operations Manager #50 verified the chicken salad, chocolate cheesecake filling, and coleslaw should all be discarded within three days after preparation. All the foods mentioned were discarded after surveyor intervention.
