Based on observation and staff interview, the facility failed to store food in a manner that protects against contamination and spoilage. This had the potential to affect all 59 residents in the facility. Census was 59.
Findings include:
Observation of the dry storage area with Dietitian #36 on 04/23/24 at 10:36 A.M., revealed a container of frosting for cakes open with no date, an open container of peanut better undated, an open, undated container of corn syrup as well as an open undated bottle of teriyaki glaze.
Interview, at the time of the observation, with Dietitian #36 confirmed all listed containers had been opened and undated and they were removed from the shelves.
Tour of the walk in freezer with Dietitian #36 on 04/23/24 at 10:47 A.M., revealed an open box of dinner roll dough, open to air; three boxes of Otis Spunkmeyer cookie dough, with dough open to air, a container of Stouffers Vegetable Lasagna that had been open on one corner and contents exposed to air. A box of frozen corn, a box of frozen carrots, a box of frozen steak hoagie patties as well as a box of tilapia that was unsecured and open to air.
Interview, at the time of the observation, with Dietitian #36 confirmed all of the listed items were open and unsecured and removed them from the shelves to be discarded.

Based on record review and staff interview, facility failed to conduct fire drills one time per quarter on each shift. The facility failed to evacuate any residents for fire drills as indicated on their fire drill evaluations. This had the potential to affect all 59 residents. Census was 59.
Findings include:
Review of fire drill logs revealed no fire drills were held on second shift for months between December 15, 2024 and April 18, 2025. There was no evidence of any drills held in October, 2024.
Interview, at the time of the review, with Wellness Director #34 confirmed the drills were not held as per regulations and confirmed there was no evidence of a fire drill held in October, 2024.
Review of fire drill records revealed no indication of attempts at evacuating residents for any of the drills performed over the past twelve months.
Interviews, at the time of the review, with Maintenance Site Lead #37 revealed he was unaware residents need to be evacuated to a safe area during a drill. Wellness Director #34 confirmed she was not aware of the need to evacuate and explained their Life Safety Director is out of town who could better explain their process.
