What the surveyor found

Based on observation, staff interview, and review of facility policy, the facility failed to store food in an appropriate manner and failed to practice appropriate hand hygiene while preparing food for resident meals. This had the potential to affect all of the residents residing in the facility. The facility census was 38 residents.

Findings include:

1.Observation on 07/07/25 at 10:48 A.M of the stand-up refrigerator revealed it contained an undated bag of blueberry muffins which were open to air.

Interview on 07/07/25 at 10:49 A.M. with Dietary Staff (DS) #3 confirmed the package of blueberry muffins in the stand-up refrigerator were undated and stored open to air.

2.Observation on 07/07/25 at 10:52 A.M. of dry storage area revealed the following open and undated items: a bag of tortilla chips, a plastic bag of peanuts, a bag of mini marshmallows, a bag of rainbow sprinkles, a bag of brown sprinkles, a bottle of hot sauce, a container of syrup.

Interview on 070/7/25 at 11:00 A.M. with DS #3 confirmed the items observed in the dry storage area were not dated or stored properly and should be discarded.

3. Observation on 07/07/25 at 11:15 A.M. of walk -in refrigerator revealed it contained the following items were stored open to air: a bag of cubed cheddar cheese, a bag of cubed pepper jack cheese, a container of provolone cheese slices, a bag of grated parmesan cheese.

Interview on 07/07/25 at 11:18 A.M. with DS #3 confirmed the items stored open to air in the walk-in refrigerator were not properly stored and should be discarded.

4. Observation on 07/07/25 at 11:24 A.M. of the walk-in freezer revealed it contained the following items: an open, undated bag of frozen breaded mushrooms, a pan of cabbage rolls exposed to air with the lid ajar, a bag of open undated frozen sliced potatoes.

Interview on 07/07/25 at 11:29 A.M. with DS #3 confirmed the frozen items in the walk-in freezer were not stored properly and should be discarded.

Review of the facility policy titled Food Storage undated revealed all refrigerated foods and frozen foods should be covered, labeled and dated. All dry storage, bulk foods where the packaging was broken or open must be stored in air-tight containers.

5. Observation on 7/07/25 at 11:30 A.M. revealed DS #2 removed meat from a package of ground beef, touching the packaging and placed the packaging in the trash, and then returning to the work area and manually mixed the meat with the same gloved hand.

Interview on 7/07/25 at 11:33 A.M with DS #2 confirmed he touched the packaging and threw it away with his gloved hands and did not remove gloves and wash his hands and don new gloves prior to touching the ground beef.

6.Observation on 07/07/25 at 11:34 A.M. of DS #4 revealed she scratched her nose with her gloved hand and then moved into the work area and touched the countertop.

Interview on 07/07/25 at 11:35 A.M. with DS #4 confirmed she scratched her nose with her gloved hand and then immediately moved to the food preparation area to do tray line.

Review of the facility policy titled Bare Hand Contact with Food and Use of Plastic Gloves undated revealed anytime a contaminated surface was touched, gloves must be changed. Gloves should be changed after touching the hair or face or after handling boxes, crates or packages.
