Based on observation, staff interview, and policy review, the facility failed to ensure food was stored in a safe and sanitary manner. This had the potential to affect all 83 residents in the facility.
Findings include:
1. Observation on 03/31/25 at 8:25 A.M. of the reach-in coolers revealed the following:
A plastic tub of yogurt, opened and partially used, with no open date
A bag of spinach with an open date of 03/20
2 packages of lunch meat, opened and not dated
A bag of hotdogs, opened, not sealed, not dated
A plastic container of cheese slices, not labeled, not dated
A bag of garlic cloves, opened, partially used, not dated
A plastic container of chickpea patties, not labeled, not dated, and the cover was not properly affixed to the container
Interview at the time of the observation, Cook #357 verified the yogurt was not dated, the spinach was outdated, lunch meat was not dated, hot dogs and cheese slices were not sealed or dated, garlic cloves were not dated, and the chickpea patties were not labeled, dated, or properly sealed.
2. Observation on 03/31/25 at 8:35 A.M. of the dry storage area revealed a bag of penne pasta, which appeared to be torn open, not sealed, and not dated.
Interview at the time of the observation, Cook #357 verified the bag of penne pasta was not sealed or dated.
3. Observation on 03/31/25 at 8:37 A.M. of the facility's walk-in cooler revealed a bag of lettuce, opened, partially used, and no open date. The lettuce was brown and had a best by date of 03/16/25. Further observation revealed a bag of parmesan cheese, opened, partially used, not sealed, and no open date.
Interview at the time of the observation, Cook #357 verified the lettuce was not dated, brown, and had a best by date of 03/16/25 and the parmesan cheese was not sealed or dated.
Review of the facility policy titled, Food Safety and Sanitation

Review of the employee training records revealed no documentation for the annual fire prevention training.
Interview on 04/03/25 at 10:33 A.M. with the Administrator verified the facility had no evidence of annual fire prevention training for staff.

Based on observation and staff interview, the facility failed to ensure trash cans in the kitchen food preparation areas were covered. This had the potential to affect all 83 residents in the facility.
Findings include:
Observation on 03/31/25 at 8:30 A.M. revealed a trash can in the food preparation area by the stove was not covered.
Interview at the same time, Cook #357 verified the trash can did not have a lid. Cook #357 stated she looked for a lid and was not able to locate one.
Observation on 04/02/25 at 11:47 A.M., the trash can in the food preparation area remained uncovered.

Based on record review, observations and staff interviews, the facility failed to ensure medications were clarified for dosage prior to administration. This affected one resident (#11) of three residents reviewed for medication administration. Facility census was 65.
Findings include:
Review of medical record for Resident #11 revealed admission date of 03/22/21. Diagnoses include Alzheimer's Disease, dementia and chronic obstructive pulmonary disease.
Review of the physician orders for Resident #11 revealed an order for Vitamin B12 daily with a start date of 02/17/23 and for Vitamin D3 on tablet daily with a start date of 09/14/23. The review of Resident #11's physician orders revealed there was no dosage identified for the Vitamin B12 or Vitamin D3 orders.
Observation on 11/22/23 at 8:09 A.M. of medication administration for Resident #11 revealed Licensed Practical Nurse (LPN) #17 administered Vitamin B12 (supplement) 1,000 micrograms (mcg) and Vitamin D3 (supplement) 400 international units (iu).
Interview on 11/11/23 at 1:20 P.M. with LPN #17 verified there was no dosage order for Resident #11's Vitamin B12 or the Vitamin D3 supplement and acknowledged the physician should be called for clarification.
Interview on 11/11/23 at 2:44 P.M. with the Director of Nursing (DON) acknowledged there were multiple dosages of both Vitamin B12 and Vitamin D3 available. The DON verified there would be no way to determine which dosage of Vitamin B 12 or Vitamin D3 should be administered to Resident #11 without the dosage being in the physician order.
