Based on observation, interview and policy review, the facility failed to ensure hair restraints and beard restraints were worn around food being prepared for the residents This had the potential to affect all 24 residents. The census was 24.
Findings include:
On 12/14/22 at 11:39 A. M. Dining Room Manager (DRM) #26, Dietary Worker (DW) #27, DW #30 and one visitor were seen in the kitchen without hair restraints. DRM #26 was observed cutting bread and working by food open to air by the cold preparation service area without a hair net or hair restraint on. DW #27 was observed at the same time walking through the kitchen from the dining room without a hair restraint or hair pulled back. DW #30 was observed walking through the kitchen with a visitor from the outside to the manager's office at 11:45 A.M. Both the visitor and DW #30 did not have hair restraints on.
On 12/14/22 at 11:39 A.M. DW #31, DW #32 and Food Service Manager (FSM) #33 were observed with facial hair beyond their chins working in the kitchen before lunch service.
FSM #33 was interviewed on 12/14/22 at 11:50 A.M. and stated beard covering depends on the length of the facial hair. but did proceed to place a beard covering on himself when corrected and agreed staff should be wearing hair restraints.
Review of Culinary policy #C_4, last revised 7/15/22, stated hairnets are to be worn while serving residents both inside the kitchen and in the dining room.
