Based on observation, interview, and review of facility policy, the facility failed to ensure foods were labeled, dated and disposed of properly. This had the potential to affect all 11 residents receiving meals from the facility kitchen. The facility census was 11.

Observation during the initial kitchen tour on 11/17/25 at 10:05 A.M. with the Administrator revealed the following concerns:
A metal container of mashed potatoes with a use-by date of 11/04/25.
Mozzarella cheese loosely wrapped in saran wrap with no open or expiration date.
A metal bowl with cooked green beans that appeared discolored with no use-by date.
Six pieces of French toast with no date on the package.
Leftover gravy with a use by date of 11/04/25.
Three bread crusts which appeared to be pizza crust open to air and unlabeled.
A metal container with what appeared to be leftover corned beef that had no use-by date.
A metal container with what appeared to be chicken and vegetable soup with no use by-date.
A metal container of what appeared to be pulled pork with no label or date.
A clear plastic container with cut vegetables (cucumbers, tomatoes, and peppers) with no use-by date which appeared to be slimy, moldy, and had an odor.

Interview on 11/17/25 at 10:25 A.M. with the Administrator confirmed the above items were not stored or discarded properly and should have been labeled and discarded by the use by date.
Review of the 02/04/21 untitled facility policy revealed all products will be stored using the first in first out method. Before use, all foods will be checked for expiration date and discarded if expired. All perishable food items that are opened and a portion of which is not used will be placed in a seal tight container and labeled with first in white label that includes the name of the item and date the package was opened. All food that is stored as leftovers will have a current and use by date placed on the item, referencing the Freezer and Refrigerator Storage Chart. Open, stored food which has a label with a date that is more than five days earlier than today's date will be discarded.
