What the surveyor found

Based on observations and staff interviews, the facility failed to ensure the facility dishwasher was properly functioning to sanitize dishes. Additionally, the facility failed to ensure a rolling gate in the kitchen was maintained in a clean and sanitary manner. This had the potential to affect all 47 residents residing in the facility. The facility census was 47.

Findings include:

1. Observation of the facility's kitchen on 07/10/25 at 8:40 A.M. revealed the facility's low temperature dishwasher had a wash and rinse temperature of 130 degrees Fahrenheit. Cook #119 was observed to test the chemical of the dishwasher with two separate chemical test strips and the sanitizer in the dishwasher was zero parts per million (ppm).

Interview with Cook #119 on 07/10/25 at 8:40 A.M. verified the dishwasher was a low temperature dishwasher that used sanitizer to sanitize the dishes. Cook #119 confirmed the facility's low temperature dishwasher had a wash and rinse temperature of 130 degrees Fahrenheit and the dishwasher's sanitizer tested at zero ppm.

2. Observation of the kitchen on 07/10/25 at 11:50 A.M. revealed there was chicken and dumplings, green beans and cauliflower sitting on the steam table in the kitchen. Further observation of the kitchen revealed there was a rolling gate behind the steam table that had gray fuzzy debris on it.

Interview with Cook #119 on 07/10/25 at 11:50 A.M. verified the rolling gate located behind the steam table had gray fuzzy debris on it. Cook #119 confirmed staff lift the gate up and down for meal service. The facility confirmed all 47 residents receive meals from the facility kitchen.
