Based on observation, staff interview, review of the facility policy, and review of operation manuals, the facility failed to maintain a clean and sanitary kitchen. This had the potential to affect all of the residents residing in the facility. The facility census was 100 residents.
Findings include:
1.Observation of the kitchen on 06/05/25 from 10:00 A.M. to 10:45 A.M. with Culinary Staff Member (CSM) #205 revealed the following undated and unlabeled items in prep station: two containers of walnuts, a container of chocolate chips, an a slice of pizza covered with plastic wrap, a container of brown sugar with a slice of bread and spoon inside, a container of melted butter, a bag of potato chips, two bags of cereal, a bag of sugar, a bag of breadcrumbs, a bag of flour. The bins of breadcrumbs, sugar, and flour bins all had debris and brownish stains on the side of the bins.
Observation of the standing refrigerator revealed the following items: an unlabeled, undated, and uncovered package of bacon, 12 cups of undated and unlabeled juice.
Observation of the coffee maker revealed the nozzles were covered with a white substance.
Observation of the deep fryer revealed there were floating crumbs and grease residue buildup.
Observation of the standing freezer revealed there was grease residue buildup on both sides.
Observation of the stove revealed there was grease buildup on the back panel and on the inside. CSM #205 opened the stove and a drop of grease from the buildup landed on her hand. There was black and white debris on the bottom of the inside of the stove.
Observation of the food warmer revealed there was grease residue buildup on the inside.
Observation of the standing freezer revealed the following opened, undated, and unlabeled items: a bag of fries, a bag of tater tots, a package of flounder, a package of sausage patties.
Observation of the dry storage revealed the following open and undated items: a bag of butterscotch chips, a bag of almonds, a container of brown sugar, a box of spaghetti, a box of penne pasta, two boxes of cake mix, a canister of dry ranch dressing mix. There was a bag of dates with an expiration date of 09/11/24.
Observation of the walk-in refrigerator revealed the following unlabeled and undated items: a bag of corn, a bag of rice.
Observation of the walk-in freezer revealed the following unlabeled and undated items: a bag of flat bread, three loaves of bread, a container of peas.
Interview on 06/05/25 at 11:50 AM with CSM #205 confirmed the following: unlabeled and undated items in the prep station, refrigerators, freezers, and dry storage, an expired item in the dry storage, white residue on the coffee maker nozzles, grease buildup on and inside the stove and on the standing freezer and the food warmer.
Review of the facility policy titled Food and Supply Storage dated January 2024 revealed unused and open food should be covered, labeled, and dated. Non-food items and supplies used in food preparation should be stored in a manner to prevent contamination.
2. Observation on 06/05/25 at 11:48 AM with CSM #210 revealed the four steam tables in the main kitchen had white and reddish-brown stains on the insides.

Review of the operator's manual for the steam table dated 10/25/19 revealed steam tables should be cleaned daily.

Review of the personnel file for the Executive Director (ED) revealed a hire date of 09/27/23 and a pre-employment physical examination dated 07/23/23
Review of the personnel file for Caregiver (CG) #27 revealed a hire date of 05/01/24 and a pre-employment physical examination dated 09/19/23.
Interview on 12/11/24 at 12:35 P.M. with the ED confirmed pre-employment physical examinations should be performed within thirty days of the hire date or upon the first day worked. The ED further confirmed the facility did not complete pre-employment physical exams for the ED and for CG #27 within the required timeframes.

Interview on 12/11/24 at 12:30 P.M. with the ED confirmed the facility had not completed an annual TB test or screening for the ED or CG #27 in 2024.
Review of the facility document titled Tuberculosis Control Plan, Residential Care Facility dated September 2006 revealed new employees were required to have a two-step TB skin test performed upon hire followed by annual TB testing or screening.

Interview on 12/11/24 at 9:38 A.M. with the ED confirmed the dirty disposable coffee mug and container should not be on the shelves in the food preparation area and the ice cream freezer needed to be cleaned.
Review of the facility policy titled Food Service and Nutrition dated September 2022 revealed the facility should store, prepare, distribute, and serve food in a way that protected it from contamination and spoilage.

1. Observation on 12/11/24 at 8:30 A.M. revealed the two dryers on the first floor had approximately one quarter inch of lint buildup in the lint traps.
Interview on 12/11/24 at 8:31 A.M with Caregiver (CG) #90 confirmed there was approximately one quarter inch of lint build up in the lint traps of each of the two dryers on the first floor.
2. Observation on 12/11/24 at 8:47 A.M revealed dryer #1 on the memory care unit had approximately one quarter inch of lint buildup in the lint trap. Dryer #2 on the memory care unit had approximately three quarters of an inch of lint build up in the lint trap.
Interview on 12/11/24 at 8:54 A.M. with CG #81 confirmed dryer #1 on the memory care unit had approximately one quarter inch of lint buildup in the lint trap, and dryer #2 on the memory care unit had approximately three quarters of an inch of lint build up in the lint trap. CG #81 further confirmed the lint traps should be cleaned daily.
