Based on record review and interview, the facility failed to ensure resident health assessments were completed as required. This affected three (#1, #5, and #6) of five records reviewed for health assessments. The facility census was 17.
Findings include:
1. Review of the medical record for Resident #6 revealed an admission date of 12/01/22 with diagnoses of seizures, constipation, Alzheimer's disease, major depressive disorder and urinary tract infection.
Review of the medical record revealed no documented evidence of annual updated dietary requirements and allergies, functional assessment, type of care and services required, self-administration evaluation and falls assessment for Resident #6.
Interview on 11/10/25 at 11:19 A.M. with the Executive Director (ED) and Director of Nursing confirmed Resident #6's medical record did not contain evidence that an annual updated dietary requirements and allergies, functional assessment, type of care and services required, self-administration evaluation and falls assessment for Resident #6 were completed.

Based on medical record review, staff interview, and review of the facility policy, the facility failed to ensure a written agreement was obtained for providing skilled services greater than 120 days. This affected two (Residents #1 and #5) of two residents reviewed on hospice services. The facility census was 17.
Findings include:
1. Review of the medical record for Resident #1 revealed an admission date of 08/17/23 with diagnoses including dementia in other diseases with behavioral disturbance, Alzheimer's Disease, and generalized anxiety disorder.
Resident #1 started hospice care service on 08/21/23. The facility was unable to provided a written agreement between facility, Resident #1 or family, physician or skilled hospice provider for receiving skilled services for greater than 120 days.
Interview on 11/10/25 at 2:00 P.M. with Executive Director (ED) verified Resident #1 has been receiving hospice services provided by Hospice Provider #300 since date of 10/24/23 and there was no written agreement has been obtained between facility, Resident #1 or family, physician or skilled hospice provider for receiving skilled services for greater than 120 days.
2. Review of the medical record for Resident #5 revealed an admission date of 11/30/23 with diagnoses including hypertension, mood (affective) disorder, dementia without behavioral disturbance, and Alzheimers disease
There was a physician's order to admit Resident #5 to Hospice Provider #300 dated 12/21/23. The facility was unable to provided a written agreement between facility, Resident #5 or family, physician or skilled hospice provider for receiving skilled services for greater than 120 days.
Interview on 11/10/25 at 2:00 P.M. with Executive Director verified Resident #5 has been receiving hospice services provided by Hospice Provider #300 since date of 12/21/23 and no written agreement has been obtained between facility, Resident #5 or family, physician or skilled hospice provider for receiving skilled services for greater than 120 days.
Review of the facility policy titled Skilled Nursing Services dated 03/18/18 revealed allowable skilled nursing care at Creekside at the Village is skilled care that is provided on a part time, intermittent basis not for more than 120 days in any 12-month period. Skilled nursing care can be provided at Creekside and Cottage by: home health agency, hospice care program, or staff members authorized under state law to provide skilled care.
