Based on record review and staff interview, the facility failed to conduct disaster drills two times per year as required. This had the potential to affect all 102 residents residing in the facility. The facility census was 102.
Findings include:
Review of the facility disaster drills revealed the facility conducted a tornado in-service on 08/08/24 which included 25 residents and 32 staff members. Further review of the facility records, revealed the facility conducted a fire safety in-service on 11/20/24 with staff and residents. Review of the documentation for disaster drills revealed no documentation to support the facility conducted disaster drills.
Interview on 07/14/25 at 11:20 A.M. with Maintenance Director #238 stated he conducted town hall in-services quarterly with residents and staff to discuss what to do during a disaster whether it was a tornado or fire. Maintenance Director #238 confirmed the facility had not conducted any disaster drills in the past year but only provided the residents and staff with disaster in-services.
