Based on review of the facility fire drills and staff interviews, the facility failed to evacuate residents in at least two fire drills a year on each shift. This had the potential to affect all 58 residents. The facility census was 58.
Findings include:
Review of the facility's fire drill records from September 2023 to August 2024, revealed residents capable of self-evacuation were not evacuated in at least two fire drills a year on each shift. Residents were evacuated on first shift on 04/29/24, residents were evacuated on second shift on 05/29/24, and residents were not evacuated on third shift.
Interview on 08/29/24 at 11:00 A.M. with Maintenance Director #100 verified residents were not evacuated in at least two fire drills a year on each shift.
