Based on review of fire drill records and interview, the facility failed to ensure fire drills were completed and documented as required. This had the potential to affect all residents residing in the facility. The facility census was 36.
Findings include:
Review of fire drill records from October 2023 to October 2024 revealed there was no evaluation of the fire drills on 10/28/23, 11/21/23, 12/12/23, 01/06/24, 02/20/24, 03/12/24, 04/02/24, 05/06/24, 06/11/24, and 07/12/24. There was no evidence of a fire drills completed for the August 2024 or September 2024. There was no evidence of third shift evacuation of residents in at least two fire drills per year on each shift.
Interview on 11/21/24 at 2:42 P.M. with Maintenance Director #807 revealed he had been hired approximately two months prior and had only completed the fire drill for October 2024.
Interview on 11/21/24 at 3:48 P.M. with the Administrator confirmed there were no fire drills completed for August and September 2024, there were no resident evacuation on third shift drills in the last year, and there were no evaluations of fire drills from 10/28/23 to 07/12/24.

Based on observation, staff interview, and policy review the facility failed to ensure foods were stored in a clean and sanitary manner to prevent contamination and food borne illness. This had the potential to affect all residents. The facility census was 30.
Findings include:
During the initial kitchen tour with Dining Services Manager (DSM) #531 on 07/18/23 between 8:32 A.M. and 8:47 A.M. the following observations were made and verified at the time of discovery.
In the walk-in freezer the following was observed:
A plastic bag of egg omelets was open, exposed to the air, and not dated.
A plastic bag of sausage patties was open, exposed to the air, and not dated and showed noticeable freezer burn.
A plastic bag of hamburger patties was open, exposed to the air, and not dated and showed noticeable freezer burn.
A box of frozen vegetables was open, exposed to air, and not dated.
A box of cod filets was open, exposed to the air, and not dated and showed noticeable freezer burn.
In the walk-in refrigerator the following was observed and verified at the time of discovery:
A box of yellow onions was in the refrigerator revealed the onions were soft and multiple onions had begun to show signs of rot.
Observation of the door outside the walk-in in refrigerator noted a laminated sheet of paper taped to the door with a red stop sign reminding staff of proper food storage practices including asking the question Is it labeled?
Review of the policy dated 10/01/14 titled Food Stock Rotation revealed any item opened must be dated with the opening date and wrapped after opening.

Based on observation and staff interview the facility failed to maintain its laundry area in a clean and safe condition. This had the potential to affect all residents. The facility census was 30.
Findings include:
Observation of the facilities laundry area on 07/20/23 at 11:15 A.M. with Quality Assurance Nurse (QAN) #990 revealed three dryers and three washers were in operation. The area behind the washer and dryer unit was noted to be full of dirt, dust debris, and various other stains. It was further noted behind each of the dryers was noticeable areas of excess lint directly touching the machines that were actively in use.
QAN #990 verified the condition of the laundry area at the time of discovery.
Interview with House Keeping Aide (HKA) #541 on 07/20/23 at 11:20 A.M. revealed she had never cleaned behind the washer and dryer areas and had never heard of anyone ever cleaning behind the machines.
