Based on interview, observation and record review, the facility failed to procure and store all food in a manner that protects it against contamination and spoilage. This could potentially affect all residents. The facility census was 39. Findings include- Observation on 04/24/25 at 10:00 A.M. with the Dining Services Manager (DSM) #8 revealed in the reach in refrigerator unit two undated bins with lettuce stored inside, two small bins containing ham with no dates on the bins, one bin of chili with no dates on the bin, one bin of pre-made sandwiches with no dates on the bin, and one bin with hot dogs with no dates on the bin. The freezer had six pieces of sausage patties open with no dates on the packaging. The dry storage area had one half of a bin with sugar with no label and a use by date of 12/26/24, one half bag of pasta opened with no use by date, and one bin of flour with a scoop inside the bin. Interview at the time of the observations with DSM #8 confirmed the items found were improperly stored and stated the foods should be labeled with dates. He also stated the scoop should not be inside the bin. Diagnoses included hemiplegia and hemiparesis following a cerebral infarction affecting left non-dominant side, aphasia following a cerebral infarction, dysphagia, and rheumatoid arthritis.
