What the surveyor found

Based on record review and staff interviews, the facility failed to ensure transmission of fire alarm signal for third (3rd) shift fire drills as required. This had the potential to affect all 43 residents residing in the facility. The census was 43.

Findings include:

Review of facility's fire and disaster drill log revealed the facility conducted 3rd shift fire drills with silent alarms on 09/30/24, 12/31/24, 03/27/25, and 06/28/25. Further review of the fire and disaster drill log revealed there was no evidence of alarm transmission records for the 3rd shift silent alarms.

Interview with Maintenance Director (MD) #124 on 07/24/25 at 3:04 P.M. confirmed the fire alarms were not tested after completing silent fire drills for the 3rd shift.

What the surveyor found

Based on review of the facility fire and disaster drill records and staff interview the facility failed to ensure residents participated in fire and disaster drills. This had the potential to affect all residents residing in the facility. The facility census was 50.

Findings include:

Review of the facility records for twelve fire drills and three disaster drills conducted by the facility in 2023 revealed there was no evidence of resident participation in the drills, including practice in evacuation of the building.

Interview on 01/04/24 at 12:02 P.M. with the Maintenance Director (MD) #94 confirmed there was no evidence of resident participation in the facility fire and disaster drills.

Interview on 01/08/24 at 12:50 P.M. with the Administrator confirmed there was no evidence of resident participation in fire and disaster drills.
