Based on staff interview and observation, the facility failed to notify the state agency of the facility closure. The facility was licensed for 20 assisted living beds.
Findings include:
Interview on 12/12/24 at 10:45A.M. with the Receptionist confirmed the facility did not have an assisted living facility anymore.
Interview on 12/12/24 at 10:53 A.M. with the Administrator confirmed the facility no longer had residents in the assisted living beds and the facility had ceased assisted living operations.
Observation on 12/12/24 at 11:00 A.M. revealed the assisted living facility was vacant. There were no residents or staff in the area.
Interview on 12/12/24 at 12:00 P.M. with the Administrator confirmed the facility did not provide written notification to the state agency of closure of the assisted living facility which ceased operations on 01/16/23.
