Based on observations, staff interviews, and policy review, the facility failed to ensure the dishwasher was washing and rinsing dishes at the proper temperatures to sanitize the dishes. Additionally, the facility failed to ensure chemical testing strips for the three compartment sink were not expired. The had the potential to affect all 91 residents in the facility. The facility census was 91.
Findings include:
Tour of the facility completed on 03/25/25 at 8:34 A.M. with Maintenance Director #215 revealed the dishwasher located in the kitchen is labeled as a high temperature dishwasher with 160 degrees Fahrenheit (F) listed as the minimum temperature for wash cycle and 180 degrees F listed as the minimum temperature for the rinse cycle.
Further observations on 03/25/25 at 8:56 A.M. revealed the dishwasher was ran with the wash temperature reading 161 degrees F and the rinse temperature reading 161 degrees F. The dishwasher was ran twice at this time with the same temperature results. The third load the temperature reading was 161 degrees F for the wash cycle and 182 degrees F for the rinse cycle. The dishwasher temperature noted to drop on the rinse cycle as soon as the dishes started to exit the machine. A fourth cycle was ran with a wash temperature of 162 degrees F and a rinse temperature of 188 degrees F.
Interview on 03/25/25 at 9:00 A.M. with Nutrition Services Manager #210 confirmed the dishwasher was not holding temperatures a couple weeks ago but she was unsure of the date and does not have access to the documents showing the dishwasher was fixed.
Follow up observation on 03/25/25 at 1:58 P.M. revealed the dishwasher wash cycle was 160 degrees F and rinse cycle was 167 degrees F. On 03/25/25 at 2:00 P.M. dishwasher wash cycle was 158 degrees F and rinse cycle was 195 degrees F. On 03/25/25 at 2:01 P.M. dishwasher wash cycle was 158 degrees F and rinse cycle was 180 degrees F. On 03/25/25 at 2:04 P.M. dishwasher wash cycle was 158 degrees and rinse cycle was 162 degrees. Nutritional Services #299 confirmed the observations.
Interview on 03/25/25 at 2:05 P.M. with Nutritional Services #299 confirmed a couple weeks ago the dishwasher was down and the kitchen used the three compartment sink chemical method checking the chemical levels with the available chemical strips. Observations of the chemical strips revealed the expiration date was 05/01/24. Nutritional Services #299 confirmed they were the same chemical test strips she used a couple of weeks ago when the dishwasher was down. Nutrition Services #299 continued to check the chemical levels in the three compartment sink at the time with the expired chemical strips. Nutritional Services #299 confirmed the chemical test strips expired on 05/01/24.
Interview on 03/25/25 at 2:08 P.M. with Nutritional Services Manager #210 confirmed the current dishwasher cycle showed wash temperature at 156 degrees F and rinse cycle at 172 degrees F and confirmed the temperatures need to be at least 160 degrees F for the wash cycle and 180 degrees F for the rinse cycle.
On 03/25/25 at 4:58 P.M. Nutritional Services Manager #210 stated Vendor #4 had just replaced the dishwasher fuse and the dishwasher was functioning at this time. Further observations on 03/25/25 at 4:59 P.M. of the dishwasher cycle with Nutritional Services Manager #210 revealed the wash cycle was 172 degrees F and the rinse cycle was 184 degrees F. Observation on 03/25/25 at 5:01 P.M. of the dishwasher cycle with Nutritional Services Manager #210 revealed the wash cycle was 170 degrees F and the rinse cycle was 170 degrees F. Nutritional Services Manager #210 confirmed the temperatures of the dishwasher were not consistently within the acceptable range to sanitize the dishes. Nutritional Services Manager #210 again confirmed Vendor #4 was in a couple weeks ago as well as Vendor #5 and they cleared the use of the dishwasher. Nutritional Services Manager #210 again confirmed she does not have paperwork showing the dishwasher was repaired by Vendor #4 or #5.
Observation on 03/25/25 at 5:20 P.M. with Nutritional Services Manager #210 and Maintenance Director #215 confirmed the dishwasher cycle completed with the wash cycle reading 170 degrees F and the rinse cycle reading 186 degrees F. Dishwasher cycle ran again at this time with the wash cycle reading 170 degrees F and the rinse cycle reading 170 degrees F. A temperature puck was place in the dish tray and ran through the dishwasher cycle at this time with the wash cycle at 163 degrees F and for the rinse cycle read at 145 degrees F. Nutrition Services Manager #210 and Maintenance Manager #215 confirmed the dishwasher temperatures were not within acceptable ranges to sanitize the dishes, which is posted on the side of the dishwasher as 160 degrees F for wash cycle and 180 degrees F for rinse cycle.
Observation on 03/25/25 at 5:23 P.M. with Nutritional Services Manager #210, picked up Vendor #5 test strips from another facility with an expiration date of 11/20/26 and tested the three compartment sink sanitation levels and the chemicals tested within the proper ranges. The facility confirmed all 91 residents residing in the facility receive their meals from the kitchen and there were currently no residents with gastrointestinal (GI) illnesses.
Review of the Sanitation and Food Safety Dishwashing by Hand policy dated 10/15/21 revealed the purpose of this policy is to describe the process used to wash dishes safely and effectively by hand if/when the dish machine is not operational. Dishes and utensils are run through the dish machine whenever possible. When it is not possible to use the dish machine, proper methods of washing dishes by hand are used. Food is rinsed from all items that are to be washed by hand. Dishes and utensils are washed, rinsed and sanitized using the 3-compartment sink method.
Review of the Kitchen Dishwasher Operations Policy, undated revealed the facility is to use thermometers for verification of proper water temperatures for models are 160 degrees F for wash cycle and 180 degrees F for rinse cycle.

Based on medical record review, observation, resident and staff interviews, the facility failed to keep the carpets in good repair to prevent potential safety hazard. This affected one (#5) of six residents reviewed for environment. The facility census was 80.
Findings include:
Review of Resident #5's medical record revealed an admission date of 07/17/20, with diagnoses including: hypertension, diabetes, macular degeneration, left ventricle hypertrophy, and bradycardia. Review of the Functional Assessment dated December 2022 revealed Resident #5 is alert and oriented and requires minimal assistance with activities of daily living.
Observation on 06/07/23 at 9:17 A.M., during tour of facility revealed torn and frayed carpet in front of Resident #5's entry door.
Interview on 06/07/23 at approximately 12:00 P.M., with Maintenance Manager (MM) #25 reported he was aware of the frayed carpet in front of Resident #5's room, and it may cause someone to trip.
Interview on 06/07/23 at 3:08 P.M., with Resident #5 reported she was disgusted with the carpet because the facility will not fix it and every time, they vacuum it gets larger.
