Based on observations, interviews, review of census list, and review of policy, the facility failed to ensure residents medications were stored properly inside the medication carts. This affected two residents (#5 and #36) whose medications had been removed from the labeled package and were placed inside plastic medicine cups labeled with the residents room numbers and stored in the top drawer of the medication cart. The facility census was 38.
Findings include:
Review of the census list verified the room numbers listed on the medication cup correlated with the rooms Resident #5 and #36 resided in.
Observation on 09/11/23 at 10:35 A.M. revealed there were two plastic medication cups located in the top drawer of the medication cart. One cup contained two pills and had Resident #5's room number written on it in black marker. The second cup contained eight pills and had Resident #36's room number written on it.
Observation and interview on 09/11/23 at 10:35 A.M., with Licensed Practical Nurse (LPN) #500 verified the two plastic medication cups located in the top drawer of the medication cart had pills in them and were labeled with resident room numbers. LPN #500 stated she had not placed the medication in the cups and verified the medications in the cup with Resident #36's room number listed on them were the medications which she had just administered to Resident #36.
Review of the undated policy titled Medication Storage, revealed medications are stored safely, securely, and properly, following manufacturer's recommendations or those of the supplier, and in accordance with federal and state laws and regulations.

Based on observations and staff interview, the facility failed to ensure open food items were labeled, dated correctly and expired foods were disposed of. This had the potential to affect 38 of 38 residents residing in the facility who received food from the kitchen. The facility census was 38.
Findings include:
Observation on 09/11/23 at 8:50 A.M., revealed there was a plastic container with corn meal inside which had a label containing a use by date of 10/26/20, a plastic container of fish batter which was not labeled with the date opened or use by date, a plastic container with powdered milk inside which had a label containing a use by date of 02/27/23, a plastic container with rice inside which had a label containing a use by date of 05/28/23, and a plastic container with popcorn kernels inside which had a label containing a use by date of 07/21/23.
Interview on 09/11/23 at 8:55 A.M., with Kitchen Employee #400, verified the plastic container of fish batter was not labeled with the date opened or a use by date. Kitchen Employee #400 further verified the plastic containers with corn meal, powdered milk, rice, and popcorn kernels had all exceeded the use by dates.

Based on record review and staff interview, the facility failed to evacuate ambulatory residents in at least two fire drills a year on each shift. This had the potential to affect 38 of 38 residents who reside in the facility. The facility census was 38.
Findings include:
Review of the third shift quarterly fire drill reports dated 09/28/22, 12/21/22, and 06/28/23 revealed the resident head count was zero for participation.
Interview on 09/11/23 at 1:45 P.M., with Director of Environmental Services (DES) #300, revealed the resident head count listed on the fire drill reports correlated with the number of residents evacuated. DES #300 additional stated he did not know residents had to be evacuated two times per shift every 12 months.
