Based on observation, staff interviews, and policy review, the facility failed to ensure staff followed infection control procedures during medication administration. This affected two (Residents #40 and #23) of two residents observed for medication administration. The facility census was 62.
Findings included:
Observation on 04/22/26 at 8:11 A.M. revealed Licensed Practical Nurse (LPN) #14 failed to wash or sanitize hands prior to handling medication. LPN #14 was observed opening the individual pill packets by pushing her painted, acrylic fingernail into the packet that contained the pill and scooping the pill with a fingernail into the medication cup of Resident #40. LPN #14 didn't wash or sanitize hands after administration.
Observation on 04/22/26 at 8:17 A.M. of LPN #14 again failed to wash or sanitize hands prior to handling medication. LPN #14 was observed opening the individual pill packets by pushing her painted, acrylic fingernail into the packet that contained the pill and scooping the pill with a fingernail into the medication cup of Resident # 23. LPN #14 didn't wash or sanitize hands after administration.
Interview on 04/22/26 at 8:32 A.M. with LPN #14 confirmed there was no sanitizer on cart and no gloves used. LPN #14 also confirmed that she was aware she didn't sanitize or wash and that it is an infection control issue.

Based on observation, staff interviews, and review of facility policy, the facility failed to ensure the kitchen stores food in a manner that protects it against contamination and spoilage. This had the potential to affect 62 residents who resided in the facility. The facility census was 62.

Observation on 04/22/26 at 8:40 A.M. found a 12-ounce, open bottle of hot sauce on dry shelf with 20 percent left and a label that stated refrigeration after opening.
Interview with Dietary Manager #39 on 04/22/26 at 8:41 A.M. confirmed the label and the need for refrigeration of the hot sauce found on the shelf.
Observation on 04/22/26 at 8:46 A.M. revealed one bag with one half an onion in it that was not dated, three separately wrapped stacks of sliced white cheese with no date, two bags of open shredded cheese, both were not dated.
Interview with Dietary Manager #39 on 04/22/26 at 8:48 A.M. confirmed the undated products in the refrigerator.
