Based on review of fire safety drills, review of alarm transcription receipts, and staff interview, the facility failed to conduct fire safety drill each shift every three months and provide a receipt of the alarm signal. This had the potential to affect all 63 residents residing in the facility.
Findings include:
Review of fire drills from December 2022 to December 2023, revealed there was not a fire drill conducted on first shift every quarter. The quarterly fire drills were not conducted on first shift for June 2023. The facility failed to provide documentation for fire drill in September 2023.
Review of the alarm transmission receipts, dated December 2022 to December 2023, revealed there were no receipts of alarm signals for fire drills conducted on 04/18/23, 07/19/23, and 10/16/23.
Interview on 12/11/23 at 1:39 P.M. with the Administrator verified first shift fire drill was not completed in June 2023, there was no facility documented fire drill in September 2023, and there were no receipts of alarm signal for fire drills conducted on 04/18/23, 07/19/23, and 10/16/23.
