Based on record review and staff interview, the facility failed to ensure residents were evacuated to a safe location for a minimum of two fire drills a year on each shift as required, which had the potential to affect all residents who resided in the facility. The facility census was 36.
Findings include:
Review of the facility documentation titled Logbook Documentation: Fire Drills revealed fire drills were completed on 11/29/23, 12/29/23, 01/29/24, 02/29/24, 03/29/24, 04/16/24, 05/30/24, 06/28/24, 07/30/24, 08/21/24, 09/27/24 and 10/16/24, and during all the fire drills, no residents were evacuated to a safe location.
Interview on 10/29/24 at 2:45 P.M. with Director of Plant Operations #304 confirmed residents were not evacuated to a safe location for any of the drills over the past 12 months and stated he wasn't aware the facility had to do anything about resident evacuations during a fire drill.
This violation is a recite to annual survey completed 12/20/22.

Based on record review and interview, the facility failed to transmit a fire alarm signal and verify receipt of that signal no more than twelve hours after a coded fire drill. This affected five of 12 fire drills reviewed. This finding had the potential to affect all 36 residents that resided in the facility.
Findings include:
Review of fire drills revealed that the coded fire drills completed on 12/07/21, 02/24/22, 03/08/22, 06/27/22, and 08/31/22 showed no evidence of alarm transmission and receipt of confirmation.
Interview on 12/19/22 at 10:16 A.M. with the Maintenance Technician verified that alarm transmission and receipt of confirmation was not completed for coded fire drills on 12/07/21, 02/24/22, 03/08/22, 06/27/22, and 08/31/22.
