Based on review of the facility fire drill forms, interview, and review of the disaster manual, the facility failed to conduct at least 12 fire drills in the past 12 months and failed to ensure a fire drill was conducted on night shift at least once a quarter (every three months) to ensure residents were familiar with a safe evacuation. This had the potential to affect all 31 residents who resided in the facility.
Findings include:
Review of the facility fire drill forms with fire drills conducted since 10/23/24 revealed no evidence the facility conducted a fire drill in September 2025 or that a fire drill was conducted every three months on the night shift.
Interview on 10/15/25 at 2:49 P.M. with Environmental Services Director (ESD) #305 confirmed that the last fire drill conducted in the facility was on 08/15/25 and there was no fire drill done in September 2025. A follow-up interview on 10/15/25 at 5:40 P.M. with ESD #305 further confirmed fire drills were to be conducted monthly.
Interview on 10/15/25 at 5:50 P.M. with the Administrator confirmed ESD #305 informed her there was a missing fire drill in September 2025 and that the document provided by ESD #305 regarding monthly fire drills was from the disaster manual.
Review of page 14, section F of the facility's Disaster Manual revealed unannounced fire drills were to be conducted monthly, and every shift should be included in a fire drill each quarter.

Based on interview, observation, and record review, the facility failed to ensure the Hydrion Quaternary (QUAT) ammonium sanitizer was dispensing at a safe concentration from the mechanical sanitizer dispenser at the three-compartment sink located in the main kitchen. This had the potential to affect all 41 residents.
Findings include:
Observations in the main kitchen on 04/04/23 at 8:55 A.M. revealed the QUAT test strips, located inside the clear hinged container mounted on the wall above the rinse bay of the three-compartment sink, expired in 2019. Staff #38 was observed entering a storage room at 8:57 A.M. and then exiting while removing plastic packaging around an unopened test strip package. The open package revealed the new strips also expired in January of 2023.
Interview on 04/04/23 at 8:59 A.M. with Staff #38 confirmed both test strips were expired. Staff #38 stated I never knew those had an expiration date.
Review of Micro Essential Laboratory, Inc, who is the manufacture of Hydrion QUAT test strips, Official Quality Control Certification, revealed in the second sentence of the third paragraph from the top under the Certification and Quality Control of Hydrion pH Test papers section, that the Hydrion papers will remain accurate until the expiration date marked on the accompanying color chart.
