Based on record review and interview the facility failed to ensure Resident #37's annual functional assessment, type of care and services, medication self-administration, fall assessment, updated nutritional requirements were performed annually. This affected one out of five residents whose medical records were reviewed for assessments. The facility census was 89.
Findings include:
Resident #37 was admitted on 08/13/17 with diagnoses including non-insulin dependent diabetes mellitus, lactose intolerance, bipolar disorder, hypertension, hyponatremia, vitamin B 12 deficiency, hyperlipidemia, psychosis, depression, and schizoaffective disorder. A review of Resident #37's record indicated the annual functional assessment, type of care and services, medication self-administration, fall assessment, updated nutritional requirements were not performed from 06/13/22 to 06/13/23.
An interview on 06/13/23 at 4:42 P.M. with Director of Nursing (DON) and Administrator verified the above findings. The DON searched for the assessments in the filing system and was unable to find the documentation the assessments were performed during the last 12 months.

Based on record review and interview the facility failed to obtain signatures on telephone orders from a physician or licensed ordering provider within 14 days from the date the order was given. This affected two residents (#2 and #37) out of five residents whose charts were reviewed for physician orders. There were 89 residents living in the facility.
Findings include:
1. Review of medical records for Resident #2 revealed an admission date of 12/14/21 with diagnoses including osteoporosis, iron deficiency anemia, hypertension, atherosclerotic heart disease, hyperlipidemia, and vitamin D deficiency.
Further review of orders in Resident #2's medical record contained the following telephone orders which were not co-signed by the physician or licensed ordering provider who originally gave the orders:
04/12/23 to follow-up with the nurse practitioner in one month
04/24/23 to cleanse the skin tear to the left forearm and to apply a non-adherent dressing every three days and as needed until resolved.
05/11/23 to follow-up with the Provider in one month
Interview with the Director of Nursing (DON) on 06/13/23 at 4:40 P.M. confirmed telephone orders taken on 04/12/23, 04/24/23, and 05/11/23 did not have a signature from the original licensed prescribing providers.
2. Resident #37 was admitted on 08/13/17 with diagnoses including non-insulin dependent diabetes mellitus, lactose intolerant, bipolar disorder, hypertension, hyponatremia, vitamin B12 deficiency, hyperlipidemia, psychosis, depression, and schizoaffective disorder. A review of Resident #37's physician orders indicated the following verbal orders obtained and from the physician were not co-signed by the physician:
On 10/20/22 an order to obtain a urinalysis with reflux culture. Immediately administer nystatin cream (cream for fungal or yeast infections) for external vaginal rash and itching.
On 05/12/22 an order for Keflex 500 milligrams (antibiotic) orally four times a day for ten days
On 06/29/22 an order for Bactrim double strength (antibiotic) administer on tablet orally twice a day until gone. Administer Mycolog cream II (anti-fungal) twice a day for two weeks to groin and panniculus.
On 04/19/22 an order to increase oxybutynin (bladder relaxant) two tablets twice a day and follow-up with the physician in the office in two weeks.
On 05/24/23 and order to resume Mycolog II cream twice a day for two weeks and then twice a day as needed for two weeks for recurrence. Decrease meal portions by 20 percent at lunch and dinner and call the office with monthly weight. Obtain a complete blood count, comprehensive metabolic panel, hemoglobin A 1 C, urinalysis, microprotein and fasting lipid panel in late June of 2023.
An interview with the DON and Administrator on 06/13/23 at 4:42 P.M. verified the above findings.

Based on observation, review veterinary records for facility pets, and interview the facility failed to ensure all pets of residents residing in the facility had an annual physical exam, including an examination to rule out parasites. The facility also failed to ensure all residents with pets kept pet vaccinations up to date. This affected two residents (#1 and #50) of six residents with pets and had the potential to affect all 89 residents residing in the facility.
Findings include:
Observation on 06/13/23 during the initial tour of the third and fourth floor which commenced at 9:00 A.M. and ended at 9:40 A.M. revealed two cats roaming through the hallway of the fourth floor, playing together at times. Further observation during the initial tour on this date indicated Resident #50 had a cat. Random observation on 06/13/23 upon completion of the initial tour revealed a small dog being walked through the hall of the first floor near the facility entrance.
Review of veterinary records indicated Resident #1 had a dog. Further review of the veterinary records for Resident #1's dog revealed the last physical examination of the pet was conducted on 02/12/20. The veterinary records also indicated the animal's rabies vaccine expired on 02/12/23.
Further review of veterinary records indicated Resident #50 had a cat. The veterinary records for Resident #50's cat revealed no evidence of a physical examination. The paperwork from the veterinarian's office dated 01/21/22 contained a list of reminders and due dates, including the following which were overdue:
12/25/17 for an intestinal parasite exam
02/28/19 for a senior blood panel
02/04/22 for a wellness exam
12/25/17 for an intestinal parasite exam
02/17/23 for every 3-year FVRCP vaccine
Re-examination of veterinary records at 4:35 P.M. for Resident #50's cat found an updated list of reminders from the veterinarian's office dated for today, 06/13/23. There were no changes in reminders or due dates from the ones listed above as documents were identical, except for the date the form was printed.
Review of the facility Pet Policy stated, an annual physical examination by a licensed veterinarian will be required, including an examination for internal and external parasites. The policy also required all pet vaccinations remain up to date.
Interview on 06/13/23 at 3:10 P.M. with Independent Living Coordinator #105 confirmed there was no evidence of physical exams for pets owned by Resident #1 and Resident #50. Additional interview with Independent Living Coordinator #105 at 4:35 P.M. on this date also confirmed the pet owned by Resident #1 had an expired Rabies vaccine and the pet owned by Resident #50 was overdue for the FVRCP vaccine. Interview at this time also re-confirmed missing physical exam reports for pet owned by Resident #1 and Resident #50.

Based on observation, record review and interview the facility failed to ensure the dishwasher rinse temperature was maintained to ensure sanitization of the dishes was achieved. This affected all the residents residing in the facility. The facility census was 89.
Findings include:
During a tour of the kitchen on 06/13/23 at 9:15 A.M. on the first floor of the facility the dishwasher was in use to wash the dirty dishes used during the breakfast meal. The dishwasher water temperature was digitally displayed on the front of the dishwasher during the dishwashing cycle. An observation of three dishwashing cycles revealed during the rinse cycle the dishwasher rinsing water temperature did not reach the 180 degree Fahrenheit to ensure the dishes were sanitized properly. The rinsing cycle achieved a water temperature of 170 to 175 degrees Fahrenheit during the three dishwashing cycles observed. On the front panel of the dishwasher was a label which indicated the dishwasher was a heat sanitization type dishwasher and the washing water temperature should reach 150 Fahrenheit and rinsing water temperature should reach 180 Fahrenheit to achieve proper sanitization of the dishware/cookware.
A review of the facility dishwashing temperature log dated 05/01/23 to 06/13/23 indicated the dishwasher water temperatures were recorded on the log during the washing and rinsing cycle three times a day after each meal was served in the facility. The dishwashing log indicated during use when washing dirty dishes the rinsing temperature did not reach 180 degrees Fahrenheit on the following days:
On 05/28/23 at 9:03 A.M. the rinsing water temperature was 174 degrees Fahrenheit.
On 05/28/23 at 12:14 P.M. the rinsing water temperature was 169 degrees Fahrenheit.
On 05/29/23 at 8:30 A.M. the rinsing water temperature was 162 degrees Fahrenheit.

An interview with Dietary Assistant #100 on 06/13/23 at 9:22 A.M. indicated he did not know what type of dishwasher system the facility used to ensure the dishwasher was working properly to ensure the dishwasher sanitized the dishes properly. Dietary Assistant #100 verified the label located on the front panel of the dishwasher indicated the dishwasher was a heat sanitization type dishwasher. The label indicated the washing temperature should reach 150 degrees Fahrenheit and rinsing temperature should reach 180 degrees Fahrenheit to ensure proper sanitization of the dishes. Dietary Assistant #100 verified the water temperature during the rinse cycle did not reach 180 degrees Fahrenheit.
An interview with Maintenance Assistant #101 on 06/13/23 at 9:24 A.M. indicated he was unaware the dishwasher temperature was not reaching a temperature of 180 degrees Fahrenheit to ensure proper sanitization of the dishes. Maintenance Assistant stated he had not received a work order or been informed the dishwasher was not working properly.
