Findings
Based on observation and interview the residence failed to ensure that qualified medication administrationpersons applied nationally recognized protocols for basic infection control and prevention when preparing andadministering medications affecting seven current residents. On 9/10/25 at approximately 8:30 a.m., during medication administration Staff #1 administered medication toResident #3 using her bare hands. Staff #1 sanitized her hands, then touched the medication cart, the door to the residents' room and her keys. Staff #1 touched two pills with her bare hand and administered them to Resident #3 who took the medication. On 9/10/25 at 12:46 p.m., the administrator stated that she expected staff to wear gloves when administeringmedication. She stated that after administering medications, she expected staff to hand sanitize.

Staff #1 was counseled on the correct procedures for hand hygiene and infection prevention during medication administration. As of 9/30/2025 all Qualified Medication Administration Personnel (QMAPs) will have received mandatory in-service training on infection control practices during medication preparation and administration. Training covered:Proper hand hygiene before, during, and after administration. Use of gloves and/or medication cups when handling medications. Avoiding direct hand-to-medication contact.-The Administrator or designee will conduct weekly audits of medication administration for 4 consecutive weeks to ensure compliance with infection control protocols.-After 4 weeks, audits will be transitioned to monthly for three additional months.-Results will be documented as party of this community’s QMP. Any noncompliance identified will result in immediate retraining and corrective action.-Ongoing competency checks will be incorporated into annual staff evaluations and retraining.
