Occurrence summary
SUMMARY OF FINDINGS:On 11/13/25, the healthcare entity investigated a reportable event. The facility acted appropriately to protect their clients by taking immediate protective actions, investigating the event, and implementing a plan to prevent a future recurrence. The healthcare entity reported neglect of a client. Client (A) experienced an assisted fall on 11/6/25 with injury noted six days later. During the course of the investigation, the healthcare entity assessed the client, notified hospice, conducted a record review and interviews. The record review showed client (A) became weak during a one person transfer with staff #1 and was assisted to the floor. Client (A) was initially assessed by a facility staff member and the client was noted to be comfortable and stable at that time. The hospice nurse was notified and conducted a visit and assessment the next day. the client reported a sharp pain to their left leg during a transfer and their pain medication was increased. The record showed the client was seen daily by the hospice nurse and continued to express increased pain and pain medication was increased each time. The client became bed bound approximately two days after the event and continued to decline. An x-ray was completed on 11/12/25 because of continued pain and it was positive for a left hip fracture. The record showed the hospice nurse educated the family regarding pain management and hospitalization and the family elected comfort care measures only based on the client’s end of life plan of care. Client (A) subsequently passed away. The healthcare entity concluded that although client (A) was diagnosed with an unfortunate fracture following their assisted fall, staff #1 followed policy and procedures at the time and neglect could not be confirmed based on their investigation. The event was not substantiated. This public occurrence summary was provided in accordance with 25-1-124, C.R.S., and was based on the report and investigation provided by the licensed healthcare entity. Department investigation of this occurrence was conducted off-site. The healthcare entity occurrence report was not submitted within the required timeframe.
