There were no deficiencies cited during an unannounced onsite State Compliance survey conducted on July 11, and 12, 2023. Based on the rules found in the Arizona Administrative Code (A.A.C.) title 9, Chapter 10, Article 6 governing a licensed Hospice Agency, the Department is approving the facility to continue operations as a Hospice Agency and to provide the following Scope of Services: Hospice Services.
