Based on observation and interview, the manager failed to ensure that a resident was treated with dignity, respect and consideration which posed a health and safety risk to the resident.

Findings include:

1. During a tour of R1's unit for the complaint investigation, E2 and the compliance officer observed the main areas of the unit and the short hall to the exit door had numerous holes in the drywall and broken wall corners. One corner where there was a corner protector, part of the corner protector was broken off. One corner was broken down to the building's frame work. The edge to the exit door had numerous gashes that gave the appearance the exit door was not in good repair. Areas of the drywall the paint was scraped off.

2. In an interview, E2 acknowledged the unit was not in good repair. The manager failed to keep the unit in good repair which gave the impression the resident was not treated with dignity, respect, and consideration.

Based on observation and interview, the manager failed to ensure the premises and equipment used at the assisted living facility was cleaned according to policies and procedures designed to prevent, minimize, and control illness or infection which posed a health and safety risk.

Findings include:

1. During a tour of R1's unit for the complaint investigation, E2 and the compliance officer observed the flooring in the main areas of the unit and the short hall to the exit door the carpet and flooring had numerous dark marks and some orange marks which gave the appearance the carpet was not kept clean.

2. In an interview, E2 acknowledged that the unit carpet and flooring did not appear clean. E2 reported a few of the marks on the carpet were stains.
