F. A manager of an assisted living home shall ensure that: 1. A fire extinguisher that is labeled as rated at least 2A-10-BC by the Underwriters Laboratories is mounted and maintained in the assisted living home;
Evidence
Based on observation and interview, the manager failed to ensure the facility's fire extinguisher was mounted. Findings include: 1. During a tour of the facility, the Compliance Officer observed a fire extinguisher sitting on the floor in the hallway right outside of a resident's room. Above on the wall was a bracket however, the fire extinguisher was not mounted in the bracket. 2. In an interview, E1 reported the maintenance person knocked the fire extinguisher off the wall approximately three days earlier and never remounted it. E1 acknowledged the fire extinguisher was not mounted and secured in a bracket.

Evidence
Based on observation, documentation review, and interview, the manager failed to ensure poisonous or toxic materials stored by the assisted living facility were maintained in labeled containers in a locked area and were inaccessible to residents. The deficient practice posed a risk to the health and safety for residents. Findings include: 1. During a tour of the facility the Compliance Officer observed the following toxic chemicals in an unlocked box marked "Emergency Kit": - "Kroger" 70% Isopropyl Alcohol; and - "Kroger" Hydrogen Peroxide. 2. The Compliance Officer observed the following sitting beside the kitchen sink: - "Great Value" Ultra Dish Liquid. 3. The Compliance Officer observed in an unlocked cabinet under the kitchen sink the following: - "Kirkland" Ultra Shine Platinum Performance dishwashing liquid. 4. In an interview, E1 reported being unaware that the toxic chemicals had been left out and unlocked.
